Building Forms in Noddle
Create lead capture forms, embed them on your website, and connect them to automations.
Forms are how Noddle captures leads from your website. When someone fills out a form, Noddle automatically creates a contact record, logs the submission, and can trigger an automation instantly — no manual work required.
Creating a Form
- Go to Sites > Forms in the left menu.
- Click + Create New Form.
- Choose to start from a blank form or pick from a template.
- The Form Editor opens. You'll see a drag-and-drop canvas where you can build your form.
Adding Fields
Click + Add Element to add fields:
- Standard fields: First Name, Last Name, Email, Phone, Address, Date of Birth, etc.
- Custom fields: Any custom field you've created in Settings > Custom Fields will be available here.
- Other elements: Text blocks, images, dropdown menus, checkboxes, file uploads, signature fields.
Drag fields up or down to reorder them. Click any field to edit its label, placeholder text, and whether it's required.
Form Settings
At the top of the Form Editor, configure:
- Form Name — internal name for your reference
- On Submit Action — what happens after submission: show a thank-you message, redirect to a URL, or open a calendar booking page
- Sticky Contact — if enabled, pre-fills the form for returning visitors whose info Noddle already has
Click Save when done.
Embedding a Form on Your Website
Once your form is built, you can place it anywhere.
- Open the form in Sites > Forms.
- Click the Integrate tab (top right).
- Choose your embed type:
- Inline — embeds directly into a page as a native element (most common)
- Popup — appears as a modal overlay on the page
- Polite Slide-In — slides in from the side subtly
- Sticky Sidebar — stays visible as the user scrolls
- Click Copy Embed Code.
- Paste the code into your website (works on GoDaddy, WordPress, Squarespace, Wix, Shopify, and most other platforms).
Using Forms in Automations
Every form submission fires a Form Submitted trigger in Noddle's Automation builder. This means the moment someone fills out your form, you can automatically:
- Send them a confirmation email or SMS
- Add a tag (e.g., "new-lead")
- Create an opportunity in your pipeline
- Assign them to a team member
- Start a full nurture sequence

Adding Forms to Calendars
You can attach a custom form to any calendar so Noddle collects additional information when someone books an appointment. Go to your calendar settings, find the Custom Form field, and select the form you want to attach. The form appears as part of the booking flow.
Tip: Forms vs. Surveys
Noddle also has a Surveys builder (same location, Sites > Surveys) for multi-step, multi-page questionnaires. Use Forms for simple lead capture. Use Surveys when you need to walk someone through multiple questions with conditional logic and branching.
